  • Health, medical and care-related information
  • Funding and eligibility details (e.g. My Aged Care or NDIS information)
  • Information about family members or representatives you nominate

We use your information to assess your needs, plan and deliver care, coordinate with health professionals and funders, meet our legal and accreditation obligations, and communicate with you.

Disclosure

We do not sell your information. We only share it where necessary to deliver your care or where required by law — for example with carers, treating health professionals, My Aged Care, the NDIS, or government bodies.

Storage and security

We store your information securely and take reasonable steps to protect it from misuse, loss and unauthorised access. Access is limited to authorised team members.

Accessing and correcting your information

You can ask to access or correct the personal information we hold about you at any time. Contact us and we will respond promptly.
